Training waiver

Please complete the waiver before your first training session. Your details are kept private and used only for club administration.

Who is this waiver for?

They get their own record at the club. You stay the contact for everything about them, and they never need an email address of their own.

Only if it's different from your first name. It's what we'll call you in class and in any email we send you.

This decides whether you sign as an adult or a guardian signs for you.

UTS students train at a discounted rate. Add your number here and the student rate applies when you join.

The number in brackets is the wearer's height that gi size is cut for. Round up or down depending on your body build. This is just so we can order kit that fits, and it is not part of the waiver. You can change it any time from your account.

So your instructors know a bit about you before your first class. This is not part of the waiver.

Is the participant prescribed any drugs which may impair reaction time or judgement?

Has the participant, within the past 5 years, suffered any blackout, seizure, convulsion, fainting or dizzy spells, or any incapacity that would render it unsafe to participate in martial arts?

Is the participant fitted with any electronic device or shunt?

Does the participant have any current physical impairment, injuries or medical conditions (for example back injuries, weak ankles)?

Is there any other medical information or health needs our instructors should be aware of for the participant's safety?

Privacy note: we collect this health information only to keep you (or the minor) safe while training.
